Overview
Physical Therapist | Skilled Nursing Facility travel assignment in Skowhegan, Maine. Start date 08/31/2026 for a 13-week contract. Skilled Nursing Facility (SNF) - Extension opportunities available
- Conduct comprehensive PT evaluations for residents and develop individualized treatment plans targeting mobility, strength, balance, gait, and range of motion.
- Implement evidence-based interventions to improve functional independence and safety with activities of daily living.
- Monitor progress, modify plans as needed, and document outcomes in the resident’s medical record.
- Provide patient and family education on exercises, safety strategies, and home programs; collaborate with the interdisciplinary team to coordinate care.
- Participate in discharge planning, referrals, and transitions of care to support continued progress.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Active Physical Therapist license in Maine or eligibility for licensure in Maine.
- Ability to work in a skilled nursing facility setting and deliver care on a travel assignment.
- Relevant experience in geriatric rehabilitation or SNF settings preferred.
- Willingness to travel for a 13-week assignment and adhere to assignment schedule (08:00–16:30, 5 days per week) with 35 guaranteed hours per week.
- Strong communication and teamwork skills; comfortable documenting care and collaborating with an interdisciplinary team.
