Rehab Liaison

At Encompass Health Corporation, we are seeking a dedicated Rehab Liaison to join our integrated healthcare services team. This role is pivotal in enhancing our rehabilitation services by acting as the primary link between healthcare providers, patients, and their families. The Rehab Liaison will ensure the seamless transition of patients through various phases of inpatient rehabilitation care by assessing patient needs, developing care plans, and coordinating with medical professionals to deliver high-quality patient care.

Responsibilities

• Develop and maintain effective relationships with hospital staff and physicians to facilitate patient referrals.
• Assess patient eligibility for admission through detailed evaluations and collaborate with medical professionals to develop care plans.
• Educate patients and families about the rehabilitation process and maintain ongoing communication to ensure understanding and satisfaction.
• Monitor patient progress and coordinate with healthcare teams to adjust treatment plans as necessary.
• Ensure compliance with all healthcare regulations and maintain up-to-date knowledge of industry practices.
