Matrix Medical Network is seeking Nurse Practitioners to perform in-home Comprehensive Health Assessments. Each visit lasts up to one hour and includes review of health history, medications, and risks to close care gaps. You’ll coordinate with PCPs, case managers, and family members, delivering services at members' homes or SNFs.
We offer flexible scheduling, competitive pay based on assessments completed, mileage reimbursement, 401(k), and ongoing education support. Sign-on bonus included.
