Social Services Assistant

Genesis HCC seeks a compassionate Social Services Assistant to support patients and families within our Healthcare & Medical Services department. In this role, you will assist licensed social workers with intake, care coordination, community resource referrals, and documentation, helping patients navigate medical and social challenges. You'll collaborate closely with an interdisciplinary care team to promote safety, compliance, and positive outcomes. Responsibilities

• Support social workers and clinical staff in coordinating patient-centered care plans
• Assist with patient intake, screenings, and gathering psychosocial histories
• Connect patients and families with community resources, benefits, and support services
• Maintain accurate documentation in electronic health records and ensure HIPAA compliance
• Collaborate with interdisciplinary care teams to enhance patient outcomes and safety
• Provide follow-up calls, appointment reminders, and basic patient education
• Help monitor patient progress and report changes in needs or risk factors
• Participate in quality, safety, and compliance initiatives within the facility
