As a Clinical Dietitian, you will provide comprehensive nutrition services across a broad spectrum of patient populations and clinical conditions.

Responsibilities include:

• Conducting comprehensive nutrition assessments and reassessments
• Developing, implementing, and monitoring individualized nutrition care plans
• Providing inpatient nutrition consultations and interventions
• Delivering outpatient medical nutrition therapy and nutrition counseling
• Educating patients and families on disease management, healthy lifestyle practices, and therapeutic diets
• Collaborating with physicians, nurses, therapists, diabetes educators, and other healthcare professionals to optimize patient outcomes
• Documenting assessments, interventions, and outcomes in the medical record
• Participating in interdisciplinary care planning and quality improvement initiatives

At Lincoln Hospital, nutrition services are embedded within the Wellness & Rehabilitation Department
, creating opportunities for rich interdisciplinary collaboration that extend beyond traditional dietitian roles. You'll work alongside skilled rehabilitation and wellness professionals, contributing to coordinated care plans that support patients throughout their health journey.
