MDS Coordinator

As part of a high‑functioning leadership and care team, conduct and coordinate the development and completion of the resident assessment in accordance with the rules and regulations of state and federal government and the policies and goals of the facility. Reports to the Director of Nursing.

Essential Job Functions
  • Assess/oversee assessment of all residents through the RAI process and ensure coding accuracy, capturing needs and level of assistance, in accordance with current rules, regulations, and guidelines.
  • Review all care plans on a regular basis for accuracy, appropriateness, and case‑mix validity.
  • Oversee and/or complete care plans for all residents according to standard nursing practice, including nursing restorative plans.
  • Establish documentation approaches and instruct nursing and ancillary staff to document properly.
  • Communicate MDS due date so that interdisciplinary team members complete their portions appropriately and on time.
  • Instruct all disciplines in proper procedures, completion of MDS items, and CAAs, Care Mix payment and quality indicators.
MDS Coordinator

Minimum Qualifications
  • MDS and ICD‑10 coding experience preferred; RAC‑CT certification preferred.
  • Current, active Maine RN licensure or current active Maine Licensed Practical Nurse (LPN).
  • Strong nursing clinical assessment, care planning, and delivery of care skills with a long‑term care population.
Skills And Abilities
  • Working knowledge of geriatric health‑care needs.
  • Clear and effective communication skills.
  • Knowledge of federal and state regulations pertaining to long‑term care and the resident assessment process.
  • Knowledge of Medicare and Maine Care regulations, as well as the application of the State Operations Manual and principles of reimbursement.
  • Knowledge of the MDS manual, updates, and quality indicator system.
  • Strong computer literacy; experience documenting within electronic medical records.
  • Ability to work and communicate tactfully with residents, staff, administration, families, physicians, ancillary services, allied health professionals, surveyors, regulators, other business contacts, the public, and the community.
  • Proficiency with Microsoft Office (Excel, Word).
