8/31/2026 Pay Rate: $1800.00 - $2700.00

POSITION DUTIES – Travels to patients’ homes within an assigned service area to conduct assessments, develop and coordinate individualized care plans, provide skilled nursing services, educate patients and caregivers, monitor progress, collaborate with physicians and interdisciplinary teams, manage documentation, and ensure continuity of care.

• ME/compact RN license
• BLS
• 2 years recent home health case management experience
• Epic and Oasis experience

Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, S/p catheters, lad draw, etc.

13 weeks

8a-5p. Every 4th rotating weekend, 3-4 days a month callback.
