LVN ​/ LPN – LTC​/SNF

LVN / LPN Position

We are seeking a qualified LVN / LPN to provide compassionate, patient-centered care in a skilled nursing facility setting. The ideal candidate will be reliable, attentive to detail, and dedicated to maintaining high standards of clinical practice.

• Administer medications and treatments accurately following physician orders.
• Monitor patient vital signs and assess patient conditions regularly.
• Document patient care activities and report any changes to the healthcare team.
• Support patients with daily activities and ensure their comfort and safety.
• Adhere to infection control policies and maintain a clean, safe environment.

• Possess a valid state LVN or LPN license.
• Demonstrate at least 1 year of experience in long-term care or skilled nursing facility.
• Maintain CPR and Basic Life Support (BLS) certifications.
• Exhibit strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.
