LVN ​/ LPN – LTC​/SNF

Long-Term Care Skilled Nursing Position

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure compassionate patient support while adhering to all relevant protocols and regulations.

Responsibilities:

• Administer prescribed medications and treatments accurately.
• Monitor and document patient conditions and responses to treatments.
• Collaborate with the healthcare team to develop and implement care plans.
• Educate patients and their families regarding health management and post-discharge care.
• Maintain compliance with facility policies, state, and federal regulations.
• Ensure proper use of PPE, including wearing a photo badge and bringing valid  required.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to demonstrate proficiency in patient assessment and documentation.
• Must have completed or be willing to complete required COVID-19 vaccinations or present validated exemptions.
• Strong communication and interpersonal skills.
• Ability to work efficiently in a fast-paced healthcare environment.
