LVN ​/ LPN – LTC​/SNF

LVN/LPN Nursing Position

Provide compassionate and proficient nursing care to residents in long-term care or skilled nursing facilities as an LVN/LPN. Ensure compliance with healthcare regulations and deliver personalized patient support.

Responsibilities:

• Administer medication and treatments accurately based on care plans.
• Monitor and assess residents' health status and report changes promptly.
• Document patient information and care activities in compliance with regulations.
• Collaborate with interdisciplinary teams to develop and implement care strategies.
• Maintain a safe and clean environment, adhering to infection control protocols.

Qualifications:

• Valid LVN/LPN license issued by the state.
• Proof of COVID-19 vaccination or valid medical/religious exemption.
• Ability to follow detailed instructions and documentation procedures.
• Strong communication and interpersonal skills.
• Previous experience in LTC/SNF settings is preferred.
