LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse

Provide licensed practical nursing care in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and compliance with healthcare standards while delivering high-quality supportive services.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient vital signs and health status regularly.
• Assist with activities of daily living, including bathing, dressing, and feeding.
• Collaborate with healthcare team to develop and implement patient care plans.
• Maintain a safe, clean, and therapeutic environment for residents.
• Comply with all facility policies, protocols, and documentation requirements.

Qualifications:

• Valid LVN or LPN license issued by the state.
• Proven experience in LTC or SNF settings preferred.
• Demonstrate excellent patient care and communication skills.
• Ability to work independently and as part of a healthcare team.
• Strong understanding of clinical practices and safety procedures.
• Must bring a government-issued  wear a photo badge at all times.
