Behavioral Health Per Diem RN; Registered Nurse

About This Role

The Behavioral Health Per Diem RN is responsible for the admission of patients for home care services, developing patient care plans, and coordinating with providers for service delivery. This role is ideal for nurses who are skilled in patient assessment and care coordination.

• Location: Guilford, CT
• Schedule: Per Diem

• Assesses/reassesses patients and establishes plans of care that are patient specific.
• Collaborates with providers and other disciplines, communicating all changes promptly.
• Assesses payment sources and ensures ongoing authorizations as necessary.
• Educates and instructs patients/caregivers in their ongoing medical maintenance.
• Completes all documentation within 24 hours, including changes in condition.
• Evaluates patient progress and takes prompt action.
• Oversees LPNs and Home Health Aides.
• Participates in case management conferences and documents per guidelines.
• Schedules patients independently according to the individual plan of care.
• Attends all staff meetings, in-services, and other required meetings.

• Nursing Diploma or Associates Degree in Nursing required, Bachelor's in Nursing preferred.

• Minimum two (2) years of nursing experience in an acute care setting.

• Current RN License in Connecticut required.
• CPR certification required and must be maintained.
