Role:
Registered Nurse (RN) Home Health, providing personalized patient care in patients' homes.
Responsibilities:Collaborate with care teams to assess patient needs, develop and implement care plans, deliver treatment and education, and monitor patient progress. Build strong patient relationships while promoting health and independence.
Qualifications & Skills:Active Maine RN license or ability to obtain one, at least 1 year of acute care or equivalent nursing experience (home care, ED, ICU preferred), valid driver's license, strong technology skills, EMR experience preferred.
Key Duties:Manage patient caseload from admission to discharge, communicate patient conditions, advise on additional services, and ensure quality care delivery. Offer flexible scheduling options, limited weekend shifts, and manageable caseloads for work-life balance.
