LVN​/LPN LTC​/SNF

Call for Salary Rate -

Job Summary:

Provide high-quality nursing care to patients in a long-term care or skilled nursing facility setting. Support patient comfort, safety, and well-being while adhering to facility policies and healthcare regulations.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient vital signs and report any significant changes to the healthcare team.
• Document patient care activities thoroughly and in compliance with regulatory standards.
• Assist with patient assessments and develop individualized care plans.
• Collaborate with multidisciplinary team members to coordinate care.
• Ensure infection control practices are maintained and PPE protocols are followed.

Qualifications:

• Valid LVN or LPN license in the state of Maine.
• Current CPR/BLS certification.
• Previous experience in LTC or SNF environments preferred.
• Ability to communicate effectively with patients, families, and team members.
• Strong organizational and documentation skills.
• Commitment to maintaining patient safety and confidentiality.
