LVN​/LPN – LTC​/SNF

Job Summary:

The LVN / LPN provides essential nursing care in a long-term care or skilled nursing facility setting. This role ensures residents receive compassionate, high-quality clinical support while adhering to facility policies and state regulations.

Responsibilities:

• Administer medications and treatments accurately and timely.
• Monitor residents’ health status and report changes to the healthcare team.
• Follow protocols for infection control and safety procedures.
• Document patient care activities thoroughly and promptly.
• Assist with activities of daily living and provide emotional support.
• Comply with documentation and regulatory requirements, including wearing identification badges.

Qualifications:

• Hold an active LVN or LPN license in the state.
• Bring a valid state or federal-issued  wear a photo badge during shifts.
• Complete or be willing to complete the COVID-19 vaccination, or present a valid religious or medical exemption.
• Demonstrate excellent communication and interpersonal skills.
• Have prior experience in long-term care or skilled nursing facilities preferred.
• Maintain reliable attendance and professionalism.
