LVN​/LPN – LTC​/SNF

Call for Salary Rate –

Job Summary:

Provide high-quality nursing care to residents in a Long-Term Care/SNF setting. Ensure patient safety, comfort, and well-being through various clinical tasks while adhering to facility standards and protocols.

Responsibilities:

• Administer medications and treatments accurately as prescribed by healthcare providers.
• Monitor and document patients’ vital signs, health status, and response to treatments.
• Assist residents with daily activities, including personal hygiene, mobility, and nutrition.
• Collaborate with the healthcare team to develop or update care plans.
• Ensure compliance with infection control policies and maintain a safe environment.

Qualifications:

• Valid LVN or LPN license in the state of Maine.
• Prior experience in LTC/SNF facilities preferred.
• Strong knowledge of clinical practices and patient care standards.
• Ability to communicate effectively with residents, families, and team members.
• Must complete required COVID-19 vaccination or have approved exemption.
• Ability to work in a fast-paced healthcare environment with attention to detail.
