Location: Fort Fairfield
Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, and community resources. You will develop and update personalized service plans, coordinate with multiple professionals, and advocate for participants to ensure safe, dignified care in their communities.
