Responsibilities
  • Promote optimal health for individuals through routine, preventive, and emergency health care; ensure work environments and Life Enhancement activities are safe, secure, and free from harm.
  • Supervise, train and coach across all settings, including Life Enhancement Program activities, to increase individuals’ self-sufficiency and independence and meet the expectations of employers, volunteer sites, families, and the individuals supported.
  • Provide education, training, counseling, guidance, and physical assistance when necessary to achieve each person’s goals in employment, education, social growth, health and wellness, communication skills, community mobility, and volunteerism.
  • Develop, write and present annual Individual Support Plans (ISP’s) and subsequent support paperwork in consultation with supervisors; advocate for implementation of ISP plans to achieve employment and program goals.
  • Perform case‑management duties including documentation of training and activities, maintenance of confidential files, and submission of required paperwork in accordance with agency procedures; attend regular meetings as needed.
  • Remain flexible to necessary changes in work schedule and tasks as assigned by supervisors to cover individual needs.
  • Maintain regular communication with individuals, ISP team members, family, guardians, advocates, and other stakeholders to facilitate consistent programming and support.
  • Act as liaison between Lifeworks, employers, volunteer sites, and community-supported sites to ensure individuals meet all community expectations.
  • Maintain confidentiality and professional demeanor with all individuals and staff.
  • Provide transportation to and from community sites using Lifeworks or personal vehicles; staff are reimbursed at the agency mileage rate.
  • Demonstrate good computer skills and willingness to learn agency computer programs for required documentation.
  • Perform other duties and/or assignments as needed.
Requirements
  • Experience working with individuals with /DD.
  • A Bachelor’s or Associate’s degree in a related field with two years of experience, or a high‑school diploma with five years of related human services experience.
  • Spanish speaking is a plus.
  • Must pass fingerprint background check, CORI, DPPC, and driving background checks.
  • Valid driver’s license required.
  • Valid MAP certification is preferred.
  • Excellent verbal and written communication skills.
