The Role:How You’ll Make an Impact
As a Case Manager, you are a trusted partner to our clients. You will support and advocate for individuals and families experiencing homelessness, helping them transition toward long-term stability and independence.
Key Responsibilities:- Advocate & Support: Build strong, respectful, and dignified relationships with clients, helping them identify their personal goals and strengths.
- Navigate Resources: Connect clients with critical agency and community resources, including housing, mental health, and social services.
- Collaborate: Partner closely with a robust network of local community support providers to coordinate care.
- Promote Independence: Guide families and individuals from crisis management to sustainable, long-term independence.
We need a kind, professional, and self-motivated individual who can work brilliantly both independently and as part of a tight-knit team.
- Education & Experience: A minimum of five (5) years of combined education and experience providing direct services in a social, health, or behavioral health field.
- A BA/BS in Social Work, Mental Health and Human Services, or a related field is strongly preferred.
- Certification: A current MHRT/C (Mental Health Rehabilitation Technician/Community) certification, or active eligibility to obtain it.
- Skills: Strong knowledge of community resources.
- Experience with resources and services for children and adolescents is a major plus.
