Homeless Services Case Manager

We are looking for a compassionate, persistent, and collaborative Case Manager to join our dedicated team. If you want to make a tangible difference in the lives of individuals and families experiencing crisis, we want to hear from you.

As a Case Manager, you are a trusted partner to our clients. You will support and advocate for individuals and families experiencing homelessness, helping them transition toward long-term stability and independence.

• Advocate & Support: Build strong, respectful, and dignified relationships with clients, helping them identify their personal goals and strengths.
• Navigate Resources: Connect clients with critical agency and community resources, including housing, mental health, and social services.
• Collaborate: Partner closely with a robust network of local community support providers to coordinate care.
• Promote Independence: Guide families and individuals from crisis management to sustainable, long-term independence.

We need a kind, professional, and self-motivated individual who can work brilliantly both independently and as part of a tight-knit team.

• Education & Experience: A minimum of five (5) years of combined education and experience providing direct services in a social, health, or behavioral health field.
• A BA/BS in Social Work, Mental Health and Human Services, or a related field is strongly preferred.
• Certification: A current MHRT/C (Mental Health Rehabilitation Technician/Community) certification, or active eligibility to obtain it.
• Skills: Strong knowledge of community resources.
• Experience with resources and services for children and adolescents is a major plus.
