Care Lync is seeking a dedicated Support Coordinator to manage individualized care plans and coordinate services for adults with intellectual and developmental disabilities under Medicaid Waiver programs. You will broker services, monitor outcomes, and help clients access housing, medical, and community supports.
Travel about 50% is required. Ideal candidates have a BS/BA in related fields, proficiency with Microsoft Office, and valid driver’s license with personal vehicle insurance.
