Children's Targeted Case Managers
to join our growing team!

As a Children's Targeted Case Manager, you'll work with children and youth from birth through age 21 who are experiencing mental health challenges and/or developmental disabilities. You'll partner with families, schools, healthcare providers, and community agencies to ensure children receive the support and services they need to succeed.

What You'll Do

As a Children's Targeted Case Manager, you will:

  • Conduct comprehensive assessments of client and family needs.
  • Develop individualized care plans and crisis plans.
  • Complete CANS assessments every 90 days.
  • Coordinate and monitor community-based services and referrals.
  • Collaborate regularly with providers, schools, healthcare professionals, and community agencies.
  • Support and empower children and families using trauma-informed, strengths-based practices.
  • Monitor client safety, well-being, stability, and progress toward goals.
  • Maintain accurate documentation and client records in compliance with agency policies, Maine Care regulations, licensing requirements, and applicable laws.
  • Manage an independent caseload, including scheduling appointments, meetings, and documentation time.
  • Participate fully in supervision, trainings, and professional development.
  • Meet a weekly expectation of
    25 billable hours
    .
What We're Looking For

Required Qualifications
  • Bachelor's degree in Psychology, Social Work, Mental Health & Human Services, Nursing, Sociology, Education, Behavioral Sciences, or a related field.
  • Strong written and verbal communication skills.
  • Reliable transportation, valid driver's license, and proof of automobile insurance.
  • Ability to travel throughout the community and provide services in clients' homes.
  • Ability to work collaboratively while managing an independent caseload.
  • Willingness to complete the required
    20 hours of annual case management training
    .
Preferred Qualifications
  • Two or more years of case management experience.
  • Knowledge of
    Section 13 of the Maine Care Benefits Manual
    .
  • Experience with Microsoft Office, Outlook, Zoom, and other virtual meeting platforms.
Working Conditions
  • Provide services in clients' homes and throughout the community in
    Somerset, Penobscot, Piscataquis, and surrounding counties
    .
  • Hybrid work environment
    with
    Wednesdays required in our Pittsfield office
    .
  • Must be comfortable working in homes where pets are present and where cigarette smoking may occur when staff are not present.
