The Social Worker (LSW) at Maine Health's Lend A Hand program supports complex care management in a primary care setting. You will address social determinants of health and help patients overcome barriers to achieve health goals.
The role requires a Maine State Licensed Social Worker, a Bachelor's in Social Work, and experience delivering therapeutic services to adults and/or pediatrics with chronic conditions. Hybrid work is available after training.
