Description
Salary: $52,000/year
Position Summary: This professional position serves as the centralized point of contact for all Multidisciplinary Staffings which include the child, family, natural and community supports, and professionals who collaborate to empower, motivate, and strengthen the family and develop an individualized plan best suited for their needs. This position will ensure staffings occur within established time frames, including all relevant team members, allows for all members to have voice and input, address child safety, permanency, and well-being.
Technical/Functional Expectations:
1. Multidisciplinary Team (MDT) Point of Contact-Essential Function:
This position is responsible for bringing together a diverse group of individuals for the purpose of allowing better engagement with families and a shared commitment and accountability. This position is responsible for scheduling, coordinating, and facilitating MDTs for all initial placements, changes in physical custody, changes in educational placement, placement decisions involving siblings, and other complex important decisions related to the child.
2. Multidisciplinary Team (MDT) Assessment:
Ensure the multidisciplinary team conducts an assessment before formulating a decision. In cases where there is not a unanimous consensus the Department will make a best interest determination and report to the court within established time frames.
3. Transition Plans:
Coordinate, track and ensure completion of transition plans for all: LOHC placements, siblings, education within the required time frames. Ensure that all transition plans are documented and uploaded in FSFN.
4. Separated Siblings:
Coordinate, schedule and review all separate siblings to assess barriers to placement together and ensure sibling visitation is occurring and contact is being maintained in other ways.
5. On Call Rotation:
Participate in the on-call rotation to provide timely placement and service delivery.
6. Customer Service:
Work with clients and families to provide support and assist with problem solving. To ensure client satisfaction.
7. Other duties as required:
Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes.
