** The Service
** Hospital in the Home (HITH) is Cabrini’s acute outreach service, providing hospital-level treatment to patients in the comfort of their own homes. The service also delivers care to Post Acute Care and Chemotherapy patients under our Cabrini at Home program.

** The Role
** We are currently seeking a Senior HITH Nurse to join our team on a part time basis 0.8 FTE basis, with consideration of alternative FTE options for the right candidate.

You’ll work across a rotating 7 day roster, including some PM/on-call shifts, to ensure seamless, high-quality care in the home setting. This senior clinical position offers the opportunity to work both autonomously and collaboratively with physicians, specialists, and other health professionals to deliver exceptional, patient-focused care.
** Key Responsibilities**:
* Develop and implement comprehensive care plans tailored to each patient’s needs
* Monitor patient progress and ensure continuity of care
* Conduct clinical assessments, administer treatments, and manage patient care
* Provide education to patients and families on health management and recovery
* Support Cabrini Health’s mission, values, and commitment to excellence
** About you
** To be successful in this role, you will possess:
* Current AHPRA registration as a Registered Nurse
* Proven acute and community nursing experience
* Strong clinical skills in wound care, IV therapy, and CVC management
* Ability to work independently and respond to clinical deterioration
* Excellent communication and documentation skills
* Commitment to quality improvement and ongoing professional development
