Human Development Services of Westchester seeks a Social Care Navigator to support adults with mental health challenges and developmental disabilities. In this role, you will assess client needs, develop care plans, and connect individuals to housing, rehabilitation, medical, and community resources. You will coordinate services with clinicians and community partners, provide advocacy and crisis support, and document progress. Responsibilities

  • Conduct biopsychosocial assessments and identify client needs, strengths, and goals.
  • Develop, implement, and monitor person-centered service and care plans.
  • Coordinate referrals to housing, rehabilitation, medical, and social support services.
  • Navigate community resources and connect clients to benefits and entitlements.
  • Collaborate with clinicians, case managers, and community providers to ensure continuity of care.
  • Provide advocacy, education, and support to empower clients toward independence.
  • Offer basic crisis intervention and de-escalation and coordinate higher-level care when needed.
  • Maintain accurate, timely documentation in electronic records and reports.
  • Track client progress, outcomes, and service utilization for quality improvement.
  • Participate in team meetings, trainings, and community outreach to enhance service access.
