  • Qualifications:
  • Graduate of an accepted Registered Nursing Program and licensed to practice nursing in the state of Virginia
  • Minimum of two (2) years experience in home health, acute care or rehabilitation setting, preferred.
  • Current BLS CPR certification.
  • Must be able to direct care appropriately to support the specific populations (adult & geriatric) provided in nursing services.
  • Experience in office procedures:
    • Ability to work independently with minimal supervision.
    • Ability to resolve problems and set priorities.
    • Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail.
    • Pleasant telephone demeanor with exceptional customer service skills.
    • Basic typing and computer skills.
    • Ability to work cohesively with others and multi-task in a busy office setting.
  • Current and active drivers license and car insurance.
  • Job Knowledge:
  • Possesses a working knowledge and understanding of the functional organizational make-up of a Home Health/Hospice Agency.
  • Working knowledge of State, Federal and ACHC guidelines/regulations (will train on the job.)
  • Working knowledge of general computer programs (ie: Word, Excel, electronic medical record charting.)
  • Basic computer usage, internet, typing, emailing, and word processing skills.
  • Working knowledge of general medical terminology.
  • Strong background in Home Health, Hospice, Medical/surgical or Rehabilitation setting preferred.
  • Chronic care management and wound care knowledge, skills and experience are considered a benefit for this position.
  • Responsibilities:
  • Works closely with the Administrator and Director of Nursing in verifying and establishing compliance for Home Health Care and Hospice Services.
  • Act as a comprehensive care coordinator for patients assigned to his/her care, assessing the patient s needs accordingly and facilitates communication amongst other clinicians providing care and the clinical supervisor:
    • Demonstrate appropriate judgment skills to be able to make independent clinical decisions in patient care matters.
    • Promote the level of clinical expertise required of staff to provide safe, exceptional and effective nursing care.
    • Follow an appropriate plan of care which addresses the patient s functional problems.
    • Identify appropriate short and long-term goals for each patient.
    • Reviews results of laboratory tests and other diagnostic procedures and reports to the physician appropriately.
    • Supervise Certified Home Health Aides (CNAs), Medical Technicians and Medical Assistants according to Federal, State, and Agency policies.
