• Registered Nurse License in the State of Virginia.
• Home Health Care Experience (minimum 2 years) required.
• COVID vaccination, updated BLS, CPR, negative PPD required.
• Prior EMR experience (Kinsser preferred).
• Efficient with documentation.

• Wound Care, Wound Vacs.
• IVs, PICC Lines.
• Tube Feeding, Tracheostomy Care.
• Foley Catheters, Ostomy Care.
• Lab Draws, Case Management.
• CPR certified.

• Patient and Family Educator:
  Educate patients and families on disease process, promote and maintain health and independence, assist in learning self‑care techniques, and advocate for patients.
• Plan of Care:
  Support patients through managing their plan of care, conducting comprehensive assessments, and collaborating with the multi‑disciplinary team to develop and implement individualized patient plans.
• Documentation:
  Complete accurate and timely documentation including OASIS assessments, admissions, re‑certifications, visits, physician orders, and care coordination.
• Collaboration:
  
  Maintain continuity of care through communication, coordination of lab results and referrals, engaging other care team members, facilitating interdisciplinary communication, participating in team meetings and case conferences, and reporting progress to a clinical manager.

Salary: $90,000.00 +. Sign‑on bonus payout will be discussed ate your own schedule and see patients at your convenience—no shifts. With one of the largest active censuses in Virginia, nurses see patients in proximity to one another keeping them near home.
