DescriptionLicensed Practical Nurse (LPN)
Join Our Team as a Licensed Practical Nurse (LPN)We are seeking a compassionate, dependable, and resident-focused Licensed Practical Nurse (LPN) to join our nursing team. In this role, you will provide direct nursing care to residents while supervising Certified Nursing Assistants (CNAs) to ensure the highest quality of care and service.
Key Responsibilities- Provide direct nursing care and monitor residents' health status.
- Observe residents daily and report significant changes in condition to the physician and interdisciplinary team.
- Administer medications safely and accurately while following infection prevention and control practices.
- Complete timely and accurate nursing documentation.
- Perform physical assessments and communicate findings appropriately.
- Develop, assign, and oversee CNA resident care assignments at the beginning of each shift.
- Supervise and support nursing staff to ensure quality resident care and compliance with facility standards.
- Assist with admissions, transfers, and discharges.
- Communicate effectively with residents, families, physicians, nurse practitioners, physician assistants, pharmacies, laboratories, and other healthcare providers.
- Participate in care planning and assist with updating residents' plans of care as needed.
- Perform wound care and other treatments as ordered by the physician.
- Review physician orders, laboratory results, and diagnostic reports and follow up as needed.
- Complete incident and accident reports and communicate concerns to nursing leadership.
- Maintain accurate medication and narcotic records and promptly report discrepancies.
- Report staffing, maintenance, housekeeping, and resident care concerns to the appropriate supervisor.
- Support the facility's commitment to Service Excellence and resident-centered care.
- Participate in committees, meetings, and special projects as assigned.
The LPN must be able to perform nursing procedures competently and professionally, including but not limited to:
- Medication administration
- Wound care and treatment documentation
- Phlebotomy
- IV therapy and central line care
- Urinary catheter insertion and management
- Tube feeding administration and management
- Oxygen therapy
- Tracheostomy care
- Airway management
- Physical and skin assessments
- Current LPN license in good standing.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and teamwork abilities.
- Ability to supervise and lead nursing support staff.
- Long-term care or skilled nursing experience preferred.

Physical Requirements:
- Prolonged periods standing, walking, working and frequently pulling, pushing, and bending.
- Must be able to lift 25 pounds at a time.
- Must be able to move 150 pounds with assistance.
