Care Manager

Position Summary:

The Care Manager oversees the coordination and quality of in-home care services for a caseload of clients. This role serves as the primary point of contact for clients and families, supervises assigned caregivers, and ensures each client's care plan is implemented safely and consistently. The ideal candidate combines strong clinical judgment with excellent communication and organizational skills.

Key Responsibilities:

• Conduct initial client assessments and develop individualized care plans in coordination with families and healthcare providers Match clients with appropriate caregivers based on needs, personality, and schedule Supervise, train, and mentor caregivers assigned to your caseload
• Conduct regular in-home visits and quality assurance check-ins with clients and families
• Monitor changes in client condition and adjust care plans accordingly Serve as the primary liaison between clients, families, caregivers, and healthcare professionals
• Respond to client and family concerns promptly and professionally Maintain accurate, timely documentation in the agency's care management system
• Participate in on-call rotation for care coordination emergencies
• Ensure compliance with state licensing regulations and agency policies

Qualifications:

• Associate's or Bachelor's degree in Nursing, Social Work, Gerontology, or related field preferred (RN, LPN, or LSW a plus)
• Minimum 2 years of experience in home care, case management, or a related healthcare setting
• Strong understanding of care planning for seniors and individuals with disabilities
• Excellent interpersonal, organizational, and problem-solving skills
• Proficiency with care management software and Microsoft Office
• Valid driver's license, reliable transportation, and current auto insurance
• Ability to pass a background check and drug screening

Physical Requirements:

• Ability to travel locally to client homes on a regular basis
• Occasional lifting/assisting up to 25 lbs during home visits
• Ability to sit, stand, and move throughout a typical workday
