Overview

Westside Care Center is looking for a Social Worker to join the team as the Memory Care Coordinator. The Memory Care Coordinator Social Worker coordinates, supervises and facilitates the delivery of social work services, as provided by facility or facility contracted staff, to all residents in accordance with applicable state and federal regulations and professional standards, with a focus on the Memory Care Program.

Responsibilities
  • Conduct face-to-face resident interviews and assessments of cognitive, mood, behavioral status in addition to other psychosocial factors.
  • Participate in planning for resident discharges.
  • Respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed.
  • Direct and supervise the delivery of medically related social services to all residents, and to their family members and significant others as needed.
  • As needed, participate in the clinical evaluation and screening of potential admissions to the facility.
  • Collaborate with the facility interdisciplinary treatment team to access information regarding, and, as appropriate, refer residents to community-based services.
  • Serve as a facility treatment team liaison with facility contracted mental health practitioners and consultants.
  • Participate in developing and implementing new behavioral and psychosocial service programs and present such programs to physicians, family members, and residents.
Qualifications
  • Bachelor’s Degree in Social Work or related human services field, required.
  • A minimum of one year’s experience in long-term care providing direct social services, preferred but not required.
