Introduction
The State of Connecticut, Department of Children and Families (DCF) is recruiting a Clinical Nurse Coordinator located at our Central Office in Manchester, CT.
Position Highlights- Full-time, 37.5 hours per week
- First (1st) shift
- Monday - Friday
- 8:30 am - 4:30 pm
- Manchester, CT
- Provide case support to social work staff as it relates to the medical and health care plan from a child's medical provider or specialist.
- Review medical plans of care from a child's medical provider/specialist and communicate care needs to social work staff and caregivers.
- Collect health & medical information to support the health component of children’s case plans.
- Assist team in collection of medical care needs to determine the level of medical complexity.
- Work collaboratively with community-based healthcare providers in facilitating care and ensuring children with complex medical needs have access to community-based services and supports.
- Assist investigation staff on the collection of medical and health information on alleged child abuse and neglect investigations.
- Participate in home visitations with social work staff as indicated.
- Attend physician appointments and hospital visits as needed.
- Assist with maintaining division health-related databases.
Working together with families and communities to improve child safety, ensure that more children have permanent families, and advance the overall well‑being of children is the central focus of the Department of Children and Families (DCF). DCF protects children who are being abused or neglected, strengthens families through support and advocacy, and builds on existing family and community strengths to help children who are facing emotional and behavioral challenges, including those committed to the Department by the juvenile justice system.

- Knowledge of nursing principles and clinical therapeutic models of individual care with emphasis on interdisciplinary team approaches; individual treatment planning including impact of medical insurance program requirements for reimbursement; structure of services for individuals and their families in institutional and community-oriented settings; aftercare facilities.
- Considerable interpersonal skills; oral and written communication skills.
Three (3) years of professional nursing experience.
Minimum Qualifications - Substitutions Allowed- A Bachelor’s degree in nursing may be substituted for one (1) year of the General Experience.
- A Master’s degree in nursing may be substituted for one (1) additional year of the General Experience.
- Experience working with children with complex medical needs.
- Experience coordinating care for the pediatric population.
- Experience reviewing health information and communicating care needs to a multidisciplinary team.
- Experience using Microsoft Office Suite, including Outlook, Teams, Word, and Excel.
- Incumbents in this class must possess and retain a license as a Registered Nurse in Connecticut or hold a Multistate or Compact Registered Nurse license.
- Incumbents in this class may be required to travel.
