JOB TITLE: LPN Primary Care Manager

EMPLOYMENT TYPE: Full-time, 40 hours/week

SCHEDULED HOURS: M-F 8AM-4:30PM or 8:30AM-5PM

Duties & Responsibilities
  • In coordination with BHH RNs, and others BHH staff assists in developing integrated behavioral health and medical treatment planning and comprehensive care management of adults and/or children with SPMI and/or chronic health conditions in CHR’s Behavioral Health Homes (BHH)
  • Provides BHH services including:
    • Assisting BHH RNs in providing Comprehensive Care Management
    • Care Coordination
    • Health Promotion
    • Comprehensive Transitional Care
    • Patient and Family Support
    • Referral to Community Support Services
    • Collaboration and coordination with medical providers and hospitals to ensure smooth transitions of care for Behavioral Health Home patients
    • Facilitates health education groups.
    • Provides training on medical diseases, treatments and medications .
    • Works with BHH Administrative System Specialist to ensure quality data management for BHH patients in order to track health related outcomes consistent with BHH outcome goals
    • Demonstrates competency in age related or specialty issues and developmental needs for each population served. Works to reduce healthcare disparities whenever possible
    • Provides individual consultation when indicated to aid clients in understanding and managing their symptoms.
    • Provides consultation when indicated to assist clients and families in learning about and managing mental illness, their medications and potential side effects
    • May performs home or outreach visits and/or client transportation
    • Keeps abreast of current ideas, theories and research regarding treatment of individuals with complex behavioral health and medical issues including factors contributing to health care disparities
    • Additional duties as required.
    QUALIFICATIONS:

    Education: Graduate of school of Nursing with LPN licensure.

    Experience:

    Experience with patients with complex medical and behavioral health issues.

    Licensure/Certification/Registration: Licensed Practical Nurse in State of CT. Valid driver’s license.

    Training: CPR & First Aid Certification required OR must obtain within 90 days…
