Job Overview
Case Manager (PC#: 1431) – Part‑time, 8 Hours. Scheduled Saturdays 3 pm‑11:30 pm. Program/
Location:
Strickland, Manchester, CT.
Strickland is an intensive residential program providing 24‑hour support services to individuals discharging from CVH.
Role OverviewCase Manager role focuses on therapeutic support, assessment, crisis intervention, and coordination of services for the target population.
Duties & Responsibilities- Utilizes appropriate therapeutic models to help clients understand and manage symptoms.
- Conducts program‑specific assessments and screens as required.
- Provides case management, collaborating with clients’ natural supports and treatment teams; connects clients to prosocial activities and offers crisis intervention and de‑escalation as needed.
- Demonstrates competency in age‑related or specialty issues and developmental needs for each population served; demonstrates program‑specific target population competencies.
- Teaches and supports learning across a broad range of skills, including emotion regulation, age‑appropriate activities, and functioning in a home‑like setting; plans and participates in activities to bolster skills and competencies.
- Documents all services and contacts with clients, collateral contacts, and appointments in compliance with agency standards.
- Transports clients as required by program.
- Participates in staff meetings, supervision, training, and other meetings as assigned.
- Completes additional duties as required.
- Bachelor’s degree in human services preferred; education plus equivalent work experience required.
- At least 2 years of experience in a mental health setting preferred.
- Valid driver’s license.
