Newport News, Virginia Hiring Range $38.60 - $53.08/Hourly Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.
The Stroke Nurse Navigator is to work in collaboration with the interdisciplinary team to compassionately serve and coordinate referrals as well as help identify high risk patients who might benefit from palliative care or hospice services. In collaboration with the team, is responsible to organize resources for timely response and support, coordinate attendance at appropriate IDT or other meetings where appropriate patients might be identified as well as maintain dashboard that would reflect key indicators for this service.
Whatyou will do
- Assesses patients and families to determine needs and available resources so that the needs of individuals are met in the most cost effective way.
- Collaborates with case management to ensure timely appropriate discharge planning needs are arranged.
- Assesses each stroke patient’s record and performs a utilization review.
- Works to continually develop and improve the stroke program.
- Develops robust relationships with the stroke patients and stroke team, works with Ad Hoc Committees.
- Works with providers and community leaders to assess resource and program needs to improve quality care across the continuum.
- Maintains and implements a patient navigation program tailored to the individualized needs of the stroke patient.
- Develops and maintains relationships with key referring and practicing physicians for the identified stroke population and continually works with physicians for care coordination.
- Works effectively and harmoniously with the stroke team and other multidisciplinary team members.
- Actively works with the stroke team to identify, plan, promote, and implement appropriate community education and awareness.
- Demonstrates knowledge of, and appropriately refers patients to the stroke clinic, and to other community resources for support and assistance.
- Provides individualized patient education, assesses their knowledge and barriers about their disease, and follows up with patient/caregivers after discharge.
- Coordinates appointment scheduling and collaborates with case management to ensure quality patient care.
- Educates patient regarding protocol for tests/appointments.
- Demonstrates strong communication and educational skills, with the ability to work with diverse populations of patients and caregivers.
- Processes physician and patient requests for tests and referrals in a timely manner.
- Responds appropriately and timely to patient calls.
- Education Bachelors Degree, Nursing (Required)
- Experience (Preferred)
- Licenses and Certifications Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
- CPR/BLS Certification
- American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required) - NIH Stroke Scale
- Test Group A (NIH) - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
