• Assess referred patients requiring rehabilitation services, providing direct patient care to determine medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals based on the patient's capacity and tolerance as directed by the physician.
• Set realistic, achievable goals for patients, document and record patient condition, and educate patients and families about appropriate physical therapy methods.

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively in the process of obtaining licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
