Who You AreAmoskeag Health is seeking a dedicated and compassionate Primary Care Nurse (RN/LPN) to provide exceptional patient care and seamless clinical support. You are confident in triaging telephone calls and walk-in patients with medical questions or concerns, ensuring timely and appropriate follow-up. You are highly organized and detail-oriented, coordinating patient communications such as lab results, medication refills, dosage changes, and provider instructions with accuracy and professionalism.

- Assess patient needs for treatment or advice via telephone or walk-in, consulting with providers as necessary to advise patients and reinforce plan of care.
- Determine if patients need to be seen by a provider and how quickly; schedule urgent appointments appropriately and promote continuity of care.
- Respond to patient calls promptly, document all calls/visits, and maintain accurate, complete records in the patient chart.
- Meet regularly with providers to establish expectations for managing team issues such as scheduling, forms, and refills.
- Integrate health teaching into daily clinical routines, including information on health maintenance and disease management for all ages.
- Coordinate care with outside facilities when referring patients to the Emergency Department or other agencies.
- Prepare prescription refills and necessary vouchers as indicated, documenting all updates.
- Maintain and attend scheduled nurse visits (e.g., injections, medication reviews, acute assessments, immunizations, BP checks) and assist with non-scheduled nursing needs as they arise (e.g., ECGs, NST setup, stabilizing acutely ill patients).
- Work cooperatively with team members to monitor schedules, assist with immunizations or rooming patients as needed, and support call volume.
- Participate in developing clinical policies/protocols and assist in processing patient paperwork (e.g., disability forms, authorizations, return-to-work letters).
- Maintain personal and clinical knowledge through education and comply with continuing education laws.
- Keep office and exam spaces clean, restock supplies, replace sharps containers, and perform opening/closing duties and lunch coverage per protocol.
- Be familiar with community resources to assist patients and attend mandatory safety and infection control trainings.
- Demonstrate sensitivity to cultural diversity and maintain age-related competence.
