In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Coos County NH

Job Overview

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The clinician will deliver personalized care in members’ homes, building trusted relationships and connecting patients to the right care.

• Conduct in‑home visits lasting approximately 45 minutes, including medication and medical history review, physical evaluation, and diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered by the member’s health plan.
• Use an iPad and the Signify Health clinical workflow for each evaluation.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Establish positive, caring interactions with patients, answering health questions and guiding them toward better self‑care.
• Maintain safety and professional conduct in diverse home environments, including settings with pets, secondhand smoke, or allergens.

• Active, unrestricted NP or PA license in the coverage area(s).
• Board certification (required).
• Ability to work in the specified primary locations:
  Coos County and surrounding area.

• Multi‑state licenses.
• Experience with in‑home clinical service delivery.

• Anticipated weekly hours: 29.5
• Pay range: $41.85 – $90.13 per hour (dependent on experience, education, geography, and other relevant factors).
