In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Cheshire County, NH

• The visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and, if ordered, additional diagnostic tests such as diabetic eye exams or spirometry.
• You will use an iPad and an intuitive clinical workflow. You are not required to prescribe medication, order lab tests, or alter the patient’s current treatment regimen.
• The role is partially remote; you will also conduct virtual visits as needed.

• Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Travel locally and within licensed states, when required, to meet patients in their homes.
• Conduct in‑home evaluations and provide diagnostic and preventive services such as spirometry, DEE, and bone density screening.
• Maintain clear communication with the Signify Health network and provide reliable, patient‑centered care.

• Active, unrestricted license(s) in the coverage area(s) of the role.
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover the following primary location:
  Cheshire County, NH.

Typical pay range: $41.85 – $90.13 per hour. The actual offer will depend on experience, education, geography, and other factors.
