Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200
-000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

• This CLC Nurse Practitioner is located in the GEC service line at the Manchester VA location
• This position is full-time at 40 hours per week
• Duties are Included but are not limited to:
  Provide comprehensive care to residents through daily medication renewals - assessments for acute and chronic conditions - wound evaluations and coordination - MDS coding - participation in clinical huddles - and supporting admission/discharge processes
• Responsibilities include weekly and bi-weekly interdisciplinary team meetings and process improvement initiatives - as well as monthly participation in quality assurance - behavior - falls - and clinical champion meetings
• Engagement in quarterly committee meetings focusing on dementia care - nurse qualification standards - and unit-based quality reporting
• Other duties as assigned
