Nurse Practitioner- CLC

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200
-000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

• This CLC Nurse Practitioner is located in the GEC service line at the Manchester VA location
• This position is full-time at 40 hours per week
• Provide comprehensive care to residents
• Daily medication renewals
• Assessments for acute and chronic conditions
• Wound evaluations and coordination
• MDS coding
• Participation in clinical huddles
• Supporting admission/discharge processes
• Weekly and bi-weekly interdisciplinary team meetings and process improvement initiatives
• Monthly participation in quality assurance - behavior - falls - and clinical champion meetings
• Quarterly committee meetings focusing on dementia care - nurse qualification standards - and unit-based quality reporting
• Other duties as assigned
