River Woods Manchester, a non-profit CCRC, is seeking a compassionate part-time Licensed Nursing Assistant/Medication Nursing Assistant to support residents in our assisted living and skilled nursing neighborhoods from 3 p.m. shift. In this role, you will provide hands-on personal care, safely administer medications under nurse delegation, monitor and report changes in resident condition, and accurately document care. You'll collaborate with nurses and interdisciplinary team members, support residents' independence and dignity, and build strong, ongoing relationships in our resident-centered, mission-driven community with growth and training opportunities.

Responsibilities

• Provide hands-on personal care and ADL support to residents
• Administer medications safely under nurse delegation
• Monitor, document, and report changes in resident condition
• Maintain accurate electronic and written documentation of care
• Follow care plans, safety, and infection control protocols
• Assist with mobility, transfers, and use of assistive devices
• Support residents' independence, dignity, and preferences
• Communicate effectively with residents, families, and team members
• Respond promptly to call lights and resident needs
• Participate in training and quality improvement initiatives
