Job Summary

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care, providing resident-centered nursing care and daily living assistance under the supervision of a registered nurse (RN). The NA delivers age-appropriate care related to the physical and psychological needs of the resident as outlined in the care plan. The role requires strict compliance with infection prevention and control policies and proper use of Personal Protective Equipment.

The NA assists with basic health care needs including bathing, toileting, grooming, dressing/undressing, vital sign collection and recording, and providing psychosocial support. The assistant helps residents transfer, reposition, and walk using correct transfer techniques and equipment, and performs range of motion and passive exercises. Responsibilities also include food preparation, serving meals and snacks, providing water and supplements as needed, and creating a positive dining experience.

The NA aids providers and nursing staff with procedures when necessary, documents resident interactions and outcomes, and communicates changes in resident condition to the charge nurse. The assistant collects non‑invasive fluid specimens, gathers vital signs, provides housekeeping and laundry services, assists with wellness and recreational programs, encourages resident participation, and may participate in departmental or organizational committees and task forces.

Qualifications

High school graduate or general equivalency diploma (GED) preferred, but not required.

Minimum age of 16. Prior clinical or nursing assistant experience in long‑term care preferred.

Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of the position start date. The facility provides CNA training and certification.

Satisfactorily completes yearly department‑specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department‑specific required certifications. When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry. Depending on facility and responsibilities, this role may also require a valid driver’s license and a vehicle report, as per Sanford Driving Policy.
