Job SummaryThe Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care in the home setting. The CCL collaborates with inter‑professional colleagues, plans, implements, and evaluates care, documents resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations. The CCL utilizes technology to enhance quality resident care, effectively delegates to and supervises unlicensed personnel, assumes overall nursing accountability for resident assignments, and serves in leadership roles on departmental and organizational task forces and committees.
The CCL demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population‑specific standards as designated in the specific care area or unit.
Must have effective communication skills to collaborate, negotiate, and resolve conflicts, situations, and/or issues with peers, inter‑professional colleagues, residents, and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations, coordinate care, perform resident assessment and education, triage, and perform other nursing interventions.
QualificationsBachelor's degree in Nursing strongly preferred. Graduate from an accredited nursing program, including those accredited by CCNE, ACEN, or NLN CNEA.
Minimum of two years of clinical experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact state. Must obtain and maintain required department‑specific competencies and certifications. Depending on location, a valid driver’s license may be required. Basic Life Support (BLS) required.
Resident Assessment Instrument (RAI) process experience preferred, but not required.
