Founding Psychiatric Mental Health Nurse Practitioner (PMHNP) – Remote
Position:
Remote (telehealth) serving Maryland, Virginia, and Washington, DC. Part‑time contract to start with a strong path to full‑time in 2027.
July to December 2026:
Program setup phase.
- Work with clinical and executive leadership to develop clinical protocols and workflows (evaluation and diagnostic standards, prescription workflow, medication transitions, referrals, collaboration model with therapists, documentation standards).
- Achieve licensure with autonomous practice authority across the full DMV area.
- Complete enrollment with Medicaid and commercial insurance payers.
January 2027 onward:
Program launch and scaling.
- Conduct initial psychiatric evaluations and establish diagnoses.
- Prescribe and manage non‑controlled medications (SSRIs, SNRIs, buspirone).
- Manage medication transitions and referrals from prior clinicians.
- Collaborate with therapists through warm handoffs and integrated care coordination.
- Maintain timely, accurate documentation in the EMR.
- Continue shaping clinical protocol development for the medication management program.
Maintain own malpractice coverage in addition to practice coverage.
Minimum Qualifications- Experience building clinical workflows and a psychiatric practice from scratch; proven ability to create evaluation and diagnostic standards, prescription and documentation workflows, payer enrollment, and collaboration models.
- Direct experience standing up or building out a psychiatric practice or program; not merely operating within an existing one.
- Early‑stage and founding experience; ability to work in an ambiguous or zero‑to‑one environment and operate without a pre‑established playbook.
- Virginia Autonomous Practice Registered Nurse (APRN) with Autonomous Practice Designation; completion of 3 years and 5,400 hours of full‑time clinical practice.
- Autonomous practice authority in Virginia, Maryland, and Washington, DC preferred;
Maryland CRNP with psychiatric specialty and DC CNP with psychiatric specialty, plus active PMHNP‑BC credentials via ANCC or AANPCB. - Maintain independent malpractice insurance.
- Commitment to LGBTQIA+ affirming care; demonstrated experience providing affirming, culturally responsive psychiatric care to LGBTQIA+ patients, including affirming language, shared decision‑making, and ongoing professional development.
- Experience providing psychotherapy during medication management visits; comfort practicing in a fully telehealth environment.
- Experience with shared medical appointments.
- Autonomous practice authority in NC, PA, DE, GA, WA, CA, NY, TX.
- Contract engagement (fee‑for‑service): $85 to $100 per hour, rate commensurate with experience and founding‑level scope.
- Strong full‑time conversion opportunity in 2027 based on patient volume and performance.
