We’re currently seeking a full‑time Hospice Medical Social Worker (MSW) who brings empathy, professionalism, and a genuine passion for making a difference. Our Hospice Medical Social Workers enjoy some excellent benefits:- Competitive Pay – $60,000 annually
- Flexible Hours – typical work hours are Monday‑Friday 8:00 AM‑5:00 PM with flexibility as needed

- Provide home‑based social services to patients dealing with end‑of‑life issues
- Work as part of a hospice team, which will include an RN, LPN, CNA, Spiritual Counselor and Hospice Aides focused on the patient’s plan of care and individual goals
- Monitor a patient’s psychosocial condition and identify social and emotional needs
- Conduct patient assessments and document progress
- Provide counseling, community resource planning, crisis intervention, and advocacy
- Consult with family and caregivers on the patient’s plan of care
- Master’s degree in social work and licensed in the state of Louisiana
- Hospice social work experience required
- CPR certification, valid driver’s license, active auto insurance, and reliable transportation
- Knowledge of state and federal home health regulations
- Strong interpersonal skills; good communicator, empathetic, compassionate, and resourceful
