Responsibilities

  • Provide skilled nursing care to patients requiring professional nursing services.
  • Assess, plan, and implement client care by following Plan of Care or Doctor's orders, which may include:
    • Dressing changes
    • Insertion and irrigation of urinary catheters
    • Oxygen administration by mask or cannula
    • Tracheostomy care and endotracheal suctioning
    • Gastrostomy tube feedings
    • Gastrostomy/Jejunum tube medication and flushes
    • SQ or IM injections
    • Insulin administration
    • Specimen collection
    • Nephrostomy tube flushes
    • Colostomy care and/or irrigation
    • Administer CPR and other emergency procedures as necessary
    • Provide staff training and in-services
  • May take the role of staff development nurse and perform episodic nurse duties in times of need.
  • Step in and perform LPN duties when needed.
  • Supervise all LPNs clinically.
  • Provide short term/short time nursing under private duty nursing or as visiting nurses for specialized services such as:
    • Diabetes Care (injection of insulin)
    • Wound care (complex wounds/wound VAC/pressure ulcers)
    • Tube feedings
    • Trach care
    • Short term services taking a few hours or less
  • Complete all nursing tasks in a comprehensive and timely manner:
    • Complete assignments efficiently and within allotted time frames.
    • Provide backup nursing coverage when the primary nurse is off duty and contact homes to determine health care needs at least weekly.
    • Maintain a current Nursing Policy and Procedure Manual; ensure its accessibility to staff within assigned homes.
    • Maintain certification in CPR.
    • Maintain current RN license.
    • Maintain certification in Infection Control for Health Care Workers.
  • Provide nursing triage for non-medically enhanced residences during off hours.
  • Screen reported symptoms, assign a level of urgency, and direct appropriate action based on standardized triage protocols.
