The Licensed Practical Nurse provides direct nursing care to residents in accordance with federal, state, and local standards and regulations. This role ensures the highest level of quality care under the guidance of the Director of Nursing Services or Nurse Supervisor.

Job Duties and Responsibilities
  • Comply with the Facility’s personnel, safety, and corporate policies and procedures.
  • Comply with nursing department’s policies and procedures.
  • Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager.
  • Treat residents, family members, visitors, and team members with dignity and respect.
  • Maintain the confidentiality of all resident and family information.
  • Report to work on time and as scheduled.
  • Assist in the admission and discharge of residents.
  • Assist residents in daily hygiene care including bathing (bed bath, shower, tub), oral, ear, eye, facial, hair and nail care.
  • Keep resident’s dry – changing clothing and linen when it becomes soiled or wet.
  • Assist residents in bowel and bladder functions; collect specimens when required.
  • Assist in transporting residents to/from appointments, activities, and social programs.
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Report and document any change observed in a resident’s condition or behavior to the Charge Nurse; assist in the care of dying residents; provide post‑mortem care.
  • Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, drainage bags, taking blood, etc., as required.
  • Perform lab tests, blood pressures, and other routine monitoring as required by the resident care plan; admit, transfer, and discharge residents as required.
  • Complete accident/incident reports as necessary.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.; complete and file required recordkeeping forms/charts upon the resident’s admission, transfer, and/or discharge; chart nurses’ notes accurately reflecting the care provided and resident response.
  • Submit accident/incident reports to the Director as required and report discrepancies in physician orders, diet changes, charting errors, etc., to the Nurse Supervisor; perform routine charting duties in accordance with established policies and procedures; maintain good working rapport with inter‑departmental personnel to ensure nursing services are adequately maintained.
  • Review complaints and grievances made or filed by assigned personnel; report appropriately to the Nurse Supervisor and follow facility procedures.
  • Receive and give the nursing report upon reporting in and at the end of shift duty hours.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies.
  • Monitor assigned personnel to ensure adherence to safety regulations in equipment and supply use.
  • Recommend equipment and supply needs to the Nurse Supervisor for the unit/shift.
  • Review care plans daily to ensure appropriate care is rendered.
  • Maintain the confidentiality of all resident care information.
  • Oversee resident needs, staff, and operations as needed.

  • Regular worksite attendance.
  • Valid professional license (LPN) in good standing with the State.
  • Valid CPR certification.
  • Basic computer skills; able to read, write, speak, and understand English.
  • Willingness to work beyond normal hours, including weekends and holidays.
  • Assist in evacuating residents during emergency situations.
  • Perform essential position functions with or without reasonable accommodation.
  • Use transfer/gait belt or assistance for lifting or carrying objects exceeding 25 pounds; awareness of required body movements.
