- Precision Medication Delivery:
Administer complex prescription and non-prescription medication regimens with 100% accuracy, strictly adhering to physician orders and state-mandated safety protocols. - Clinical
Collaboration:
Partner directly with RNs and LPNs to synchronize treatment plans, optimize medication schedules, and elevate the overall standard of patient care. - Advanced Monitoring:
Act as the "front line" for resident safety—proactively identifying, documenting, and reporting adverse reactions or subtle changes in clinical status to supervising nurses. - Rigorous Documentation:
Maintain "survey-ready" Medication Administration Records (MARs), ensuring every dosage, refusal, and observation is captured with real-time accuracy. - Safety & Security:
Manage the strict chain of custody for all medications, ensuring secure storage and adherence to all facility and federal regulatory guidelines.
- CNA Credential (Required):
Must hold an active Certified Nursing Assistant certificate in good standing with the State of Wisconsin. - Medication Assistant Certification (Required):
Must hold an Accredited Medication Assistant Certification active in Wisconsin. - Expertise:
Strong command of medical and pharmacological terminology and safe administration techniques. - Experience:
Prior experience in Skilled Nursing (SNF) or Long-Term Care (LTC) is highly prioritized. - Soft Skills:
Exceptional communication skills for high-stakes interaction with residents, families, and medical staff.
