Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home, assisted living, or wherever patients call home. You will assess patients, develop plans of care, coordinate with IDG, educate families, and ensure timely documentation while upholding regulatory standards.
Minimum 1 year hospice or RN experience;
Wisconsin RN license; CPR; valid driver’s license. This is a full-time, on-site role supporting end-of-life care in Wisconsin.
