Position SummaryWe are seeking a motivated Nurse Practitioner (AGNP or CNP) to deliver on‑site care to senior living communities. If you’re passionate about serving older adults and want to expand into wound care, this is your chance. We’ll even pay for your wound care certification if you’re not already credentialed.
Key Responsibilities- Deliver compassionate primary care in senior living communities
- Develop and manage care plans, provide wound care, and support chronic disease management
- Coordinate with interdisciplinary teams, families, and support staff
- Keep documentation current and clear in EMR systems
- Current CNP license in Minnesota
- AGNP certification preferred
- Geriatric or primary care experience preferred
- Reliable transportation and valid auto insurance
- Strong communication and clinical judgment
